Non-intubated video-assisted thoracoscopic surgery (NIVATS) offers faster recovery and shorter anesthesia times compared to intubated VATS. However, NIVATS may increase the risk of postoperative radiologic complications.
Area of Science:
- Thoracic Surgery
- Anesthesiology
- Minimally Invasive Surgery
Background:
- Limited data exists on non-intubated anesthesia for spontaneous breathing surgeries.
- Previous studies often involved small cohorts, diverse surgical types, and lacked intubation controls.
Purpose of the Study:
- To compare perioperative conditions and postoperative recovery between non-intubated video-assisted thoracoscopic surgery (NIVATS) and intubated VATS (IVATS).
- To evaluate the safety and efficacy of NIVATS in lobectomy procedures.
Main Methods:
- Retrospective comparison using propensity score-matched analysis.
- 119 patients in the NIVATS group were matched with patients in the IVATS group who underwent lobectomy.
Main Results:
- NIVATS group demonstrated significantly shorter operative and anesthesia times (P<0.01).
- Faster and more stable recovery in the post-anesthesia care unit (PACU), shorter PACU stay times, and reduced sedative/analgesic use were observed in NIVATS (P<0.05).
- Higher incidence of pulmonary exudation, atelectasis, and pleural effusion in NIVATS (P<0.05), with similar postoperative oxygenation and blood parameters.
Conclusions:
- NIVATS facilitates a more rapid and stable PACU recovery with no significant impact on oxygenation.
- NIVATS is associated with an increased likelihood of postoperative radiologic complications.
