    Background:

    • Corneal inlays are implanted to correct refractive errors.
    • LASIK flaps are used to create a pocket for inlay insertion.

    Observation:

    • Two patients developed flap melting and corneal opacification years after inlay implantation for hyperopic correction.
    • This complication led to a sudden loss of corrected distance visual acuity (CDVA).

    Findings:

    • Flap melting occurred 9 and 11 years postoperatively in the two patients.
    • Explantation of the corneal inlays resulted in the recovery of CDVA to 1.0 in both cases.

    Implications:

    • Corneal inlays may present delayed biocompatibility issues.
    • Long-term monitoring is essential for patients with corneal inlays.
    • Surgical intervention may be required to manage inlay-related complications.
