Background:

  • Primary central nervous system lymphoma (PCNSL) is a rare non-Hodgkin lymphoma, typically presenting as a solitary lesion within the brain or spinal cord.
  • Cerebellar involvement in PCNSL is uncommon, occurring in only 9% of cases.
  • The incidence of PCNSL is increasing in both immunocompromised and immunocompetent individuals.

Observation:

  • A 71-year-old immunocompetent male presented with acute dizziness, nausea, vomiting, and gait imbalance.
  • Contrast-enhanced CT revealed three intensely enhancing masses in the right cerebellar hemisphere.
  • Diagnostic imaging excluded a primary extracranial tumor.

Findings:

  • Histopathology confirmed diffuse large B-cell lymphoma, non-germinal center type.
  • This represents the first reported case of PCNSL presenting as multiple cerebellar masses.
  • The patient underwent surgical resection of the cerebellar lesions.

  • The rising incidence of PCNSL underscores the need for increased awareness and timely diagnosis.
  • Aggressive PCNSL requires prompt diagnosis and intervention for optimal patient outcomes.
