Summary

Cone-beam computed tomography (CBCT) shows promise for diagnosing conductive hearing loss in adults with intact tympanic membranes. This low-radiation imaging technique offers reliable results, though further studies are needed to confirm its superiority over multi-detector computed tomography (MDCT).

Background:

  • Conductive hearing loss with an intact tympanic membrane requires accurate radiological assessment.

Purpose of the Study:

  • To compare the diagnostic value of cone-beam computed tomography (CBCT) with MDCT for adult conductive hearing loss.
  • To evaluate inter- and intraobserver variability and radiation dosimetry for both techniques.

Main Methods:

  • A prospective, single-center study included ten adult patients.
  • Patients underwent both MDCT and CBCT of the temporal bones.
  • Diagnostic agreement was assessed using the kappa statistic; radiation dose was compared via CT dose index (CTDI) ratios.

Main Results:

  • Satisfactory diagnostic agreement (kappa = 0.69) was found between MDCT and CBCT.
  • Acceptable inter- and intraobserver agreement was observed.
  • MDCT delivered an average of 4.01 times higher radiation dose than CBCT.

Conclusions:

  • CBCT is a reliable, low-radiation imaging method for evaluating conductive hearing loss in adults.
  • While promising for middle ear pathologies like otosclerosis, CBCT's superiority over MDCT requires further investigation.
