Background:

  • Chylothorax, the accumulation of chyle in the pleural space, is a rare complication (<1%) of thoracic surgery and even rarer after abdominal surgery.
  • Hepatectomy, a major abdominal surgery, carries a low but significant risk of developing postoperative complications like chylothorax.

Observation:

  • A 74-year-old woman developed chylous ascites and subsequent massive pleural effusion after undergoing extensive surgery for hilar cholangiocarcinoma.
  • Initial conservative management including pleural drainage, nutritional support, and octreotide was insufficient to resolve the persistent chylothorax.

Findings:

  • Lymphangiography with Lipiodol identified lymphatic extravasation into the pleural space, aiding diagnosis despite an unidentifiable leak source.
  • Following lymphangiography and chest tube placement, the chylothorax significantly diminished, allowing for patient discharge and long-term recovery.

Implications:

  • This case suggests that chylothorax should be considered in patients presenting with pleural effusion post-hepatectomy.
  • Lymphangiography may serve as a valuable diagnostic and therapeutic tool for managing chylothorax following major abdominal surgery like hepatectomy.
