Hepatocellular carcinoma (HCC) in normal liver is rare. Ultrasonography (US) can help diagnose these tumors by identifying specific features like lateral shadowing and characteristic enhancement patterns on contrast-enhanced US (CEUS).
Area of Science:
- Hepatology
- Diagnostic Imaging
- Oncology
Background:
- Hepatocellular carcinoma (HCC) typically arises in the setting of chronic liver disease.
- HCC occurring in a non-cirrhotic, normal liver is exceptionally uncommon.
Purpose of the Study:
- To investigate the clinical features of HCC in normal liver.
- To evaluate the diagnostic utility of ultrasonography (US) for HCC in normal liver.
Main Methods:
- Retrospective analysis of clinical data and US findings.
- Inclusion of 12 cases of surgically resected HCC in normal liver.
Main Results:
- Patients were largely asymptomatic with normal liver function and no known risk factors.
- HCCs were often large, encapsulated, solitary, and well-to-moderately differentiated.
- US revealed a hypoechoic rim and lateral shadowing; contrast-enhanced US (CEUS) showed arterial hyperenhancement with postvascular washout.
Conclusions:
- Ultrasonography can raise suspicion for HCC in normal liver.
- Key US findings include lateral shadowing and hypervascularity on CEUS, suggesting peritumoral capsule formation.
