Background:

  • Management of ruptured type B aortic dissection (TBAD) presents significant challenges despite advances in endovascular techniques.
  • Chronic TBAD with rupture requires urgent intervention to prevent catastrophic outcomes.

Observation:

  • A patient with chronic TBAD presented with massive hemothorax and shock due to thoracic aortic aneurysm rupture.
  • Initial thoracic endovascular aortic repair (TEVAR) achieved primary entry tear sealing but persistent retrograde false lumen flow was observed.

Findings:

  • Aortography confirmed retrograde flow into the false lumen via a distal re-entry tear, necessitating further intervention.
  • A modified candy-plug device was deployed in the false lumen to occlude retrograde flow and decompress the ruptured segment.
  • Successful occlusion of retrograde flow was achieved, stabilizing the patient.

Implications:

  • This case highlights the utility of a modified candy-plug device as a rescue therapy for complex ruptured TBAD with persistent false lumen flow.
  • The findings suggest this technique may be valuable in managing challenging endovascular repair failures in aortic dissection.
  • Further investigation into modified candy-plug devices for aortic dissection management is warranted.
