Background:

Verrucous carcinoma is a rare, low-grade squamous cell malignancy with high recurrence risk.
Perianal verrucous carcinoma lacks established management guidelines due to its infrequent occurrence.

Purpose of the Study:

To highlight diagnostic challenges and management considerations for perianal verrucous carcinoma.
To emphasize the importance of biopsy for persistent lesions unresponsive to standard treatments.

Main Methods:

Case report of a Human Papilloma Virus-negative perianal verrucous carcinoma in a 45-year-old female.
Clinical presentation mimicked a persistent genital wart unresponsive to local therapy.
Histopathological examination post-excision revealed lichen sclerosus inflammatory changes.

Main Results:

The tumor presented as a persistent genital wart, initially treated with standard dermatologic therapy.
Recurrence and misdiagnosis are significant concerns due to the tumor's low-grade nature and mimicry of benign conditions.
Lichen sclerosus inflammatory changes were noted near the excision scar.

Conclusions:

Persistent genital warts unresponsive to treatment necessitate biopsy to rule out verrucous carcinoma.
Regular follow-up (3-6 months) is recommended due to the high risk of relapse.
Clarifying etiology and classification of verrucous carcinomas remains an area for further research.
