Area of Science:

  • Public Health
  • Maternal and Child Health
  • Healthcare Disparities

Background:

  • Improved pregnancy outcomes are linked to prenatal care, yet disparities persist.
  • Subpopulations continue to receive inadequate or less than adequate prenatal care.

Purpose of the Study:

  • To examine predictors of prenatal care utilization and adequacy.
  • To identify specific subpopulations at risk for inadequate prenatal care.

Main Methods:

  • Stratified random sampling of mothers from state birth certificate files.
  • Mailed questionnaire survey to assess prenatal care utilization.
  • Analysis of financial coverage, maternal/paternal/family attributes, and healthcare system characteristics.

Main Results:

  • Deficiencies in prenatal care utilization were concentrated in specific areas and subpopulations.
  • Most women did not maintain the recommended schedule of prenatal care visits.
  • Younger, less educated, low-income, rural residents, and Medicaid recipients were less likely to receive adequate care, even after controlling for need.

Conclusions:

  • Economic and non-economic barriers to prenatal care utilization must be distinguished for policy development.
  • Medicaid receipt does not guarantee adequate prenatal care use.
  • Geographic location is a significant predictor of prenatal care adequacy.
