Summary

Cholesterol crystal microembolisation after angiography, known as multiple cholesterol emboli syndrome, is rare but causes irreversible organ damage. This condition carries a poor prognosis and requires better therapeutic options.

Background:

  • Multiple cholesterol emboli syndrome (MCES) is a known complication of atherosclerosis.
  • While often spontaneous, MCES following angiography is infrequently reported.

Observation:

  • Three cases of widespread microembolisation of cholesterol crystals post-angiography are presented.
  • A review of the literature identified only 19 previously reported cases.

Findings:

  • Dissemination of cholesterol microemboli can lead to irreversible organ ischaemia.
  • Risk factors include male gender and pre-existing widespread atherosclerosis.
  • The prognosis for MCES is generally poor.

Implications:

  • Further research is needed to improve diagnostic and therapeutic strategies.
  • Patient risk stratification for angiography should consider atherosclerosis severity.
