Abnormal Pouchogram Predicts Pouch Failure Even in Asymptomatic Patients.

Background:

• Anastomotic complications after restorative total proctocolectomy with ileal pouch-anal anastomosis (IPAA) for ulcerative colitis can negatively impact functional outcomes and quality of life.
• These complications may lead to pouch failure, necessitating pouch excision or permanent diversion.
• Routine contrast enema (pouchography) is used to assess anastomotic integrity before ileostomy reversal, but its clinical utility is debated.

Purpose of the Study:

• To evaluate the association between preoperative clinical characteristics, abnormal pouchography findings, and the occurrence of long-term pouch complications.
• To identify predictors of pouch failure in patients undergoing IPAA for ulcerative colitis.

Main Methods:

• Retrospective chart review of ulcerative colitis patients who underwent IPAA and pouchography before ileostomy closure at a tertiary care center (2000-2010).
