Summary

This study describes the characteristics and treatments of autoimmune interstitial lung diseases (Ai-ILD) in Latin America. Most patients were women with connective tissue disease-associated ILD, often treated with steroids and cyclophosphamide.

Background:

  • Limited data exists on the prognosis and natural history of autoimmune interstitial lung diseases (Ai-ILD).

Purpose of the Study:

  • To evaluate the clinical characteristics of patients diagnosed with Ai-ILD in Latin America.
  • To describe the treatment strategies employed for Ai-ILD in the region.

Main Methods:

  • An ambispective, multicenter cohort study was conducted across 25 centers in Argentina, Colombia, and Uruguay.
  • Patients were diagnosed with Ai-ILD by a multidisciplinary team and classified into subgroups: ILD-CTD, IPAF, and ILD-ANCA.
  • Thoracic images were reviewed by a blinded radiologist.

Main Results:

  • 381 patients were included; 74% were women, with a mean age of 58 years.
  • The majority (85.1%) were classified as ILD-CTD, primarily associated with rheumatoid arthritis, systemic sclerosis, and dermatomyositis.
  • Mild decrease in forced vital capacity was observed in 50% of patients; common treatments included steroids with cyclophosphamide (30.1%) and azathioprine (20.3%).

Conclusions:

  • This is the first study characterizing Ai-ILD patients and their treatments in Latin America.
  • ILD-CTD is the predominant subtype, with varied treatment approaches observed.
  • Further research is needed to assess prognosis and the impact of current treatments on Ai-ILD outcomes.
