Laparoscopic magnetic sphincter augmentation (MSA) effectively treats refractory gastroesophageal reflux disease (GERD). This study shows durable symptom improvement and low dysphagia rates in patients undergoing MSA.

Background:

  • Gastroesophageal reflux disease (GERD) significantly impacts quality of life.
  • Medical management is often insufficient for severe GERD.
  • Laparoscopic magnetic sphincter augmentation (MSA) is an option for refractory GERD.

Purpose of the Study:

  • To evaluate the outcomes of laparoscopic magnetic sphincter augmentation (MSA) for GERD.
  • To assess symptom improvement and quality of life changes post-MSA.
  • To analyze complication rates and long-term durability of MSA.

Main Methods:

  • Retrospective review of 98 patients undergoing laparoscopic MSA for GERD (2012-2016).
  • Assessment of symptoms using the GERD-Health-Related Quality of Life (GERD-HRQL) questionnaire.
  • Objective testing included pH monitoring, manometry, endoscopy, and upper GI series; median follow-up was 46 months.

Main Results:

  • Significant reduction in GERD-HRQL scores from 25 preoperatively to 8 (short-term) and 5 (long-term) (P < .05).
  • Esophagitis present in 18%, hiatal hernia in 59%; 16% required full hiatal dissection.
  • Low complication rate; 5 patients (5%) required reoperation, including one device erosion. No intraoperative complications.

Conclusions:

  • Laparoscopic MSA demonstrates excellent efficacy for GERD treatment.
  • Symptom improvement is significant and durable long-term.
  • The procedure is associated with a low rate of new-onset dysphagia.
