Background:

  • Necrotizing myositis (NM) is a rare and aggressive soft tissue infection of muscle.
  • Unlike other necrotizing infections, NM can present subtly before rapidly progressing to a life-threatening state.
  • High mortality rates necessitate prompt diagnosis and surgical intervention.

Observation:

  • A 74-year-old male with a history of prostate cancer presented with urosepsis after a fall.
  • He developed severe groin pain and swelling, with CT revealing gas in the bilateral thighs, indicative of necrotizing infection.
  • Surgical exploration confirmed necrotizing myositis.

Findings:

  • Cultures from the thigh and bladder identified Streptococcus intermedius and Clostridium clostridioforme.
  • These bacteria, along with Peptostreptococcus, suggest a potential infectious source from the prostate or pubic symphysis.
  • This is the first reported instance of S. intermedius and C. clostridioforme causing NM.

Implications:

  • This case underscores the importance of a high index of suspicion for rare conditions like NM.
  • Early recognition and aggressive surgical management are critical for improving patient outcomes and reducing mortality.
  • Identifying specific bacterial culprits aids in understanding disease pathogenesis and guiding treatment strategies.
