• She experienced progressive dysmenorrhea and right leg/groin pain during menstruation.
• Pelvic Magnetic Resonance Imaging (MRI) was performed.

Findings:

• MRI revealed extensive pelvic endometriosis.
• A distinct lesion was identified within the right round ligament, extending into the inguinal canal.
• This indicates an unusual extragenital site for endometriosis.

Implications:

• This case expands the understanding of endometriosis presentation.
• Inguinal endometriosis can be a cause of pelvic pain and subfertility.
• Consideration of endometriosis in the inguinal region is important for diagnosis and treatment.
