Summary

Hypertensive disorders of pregnancy (HDP) and preeclampsia (PE) are linked to obesity. Visceral fat mass (VFM) may be a better predictor of PE risk than BMI alone, as VFM-associated adipokines were elevated in PE cases regardless of BMI.

Background:

  • Hypertensive disorders of pregnancy (HDP), including preeclampsia (PE), pose significant maternal health risks.
  • Obesity is a major risk factor for HDP, but body mass index (BMI) alone has limited predictive power.
  • Increased visceral fat mass (VFM) is linked to hypertension in nonpregnant individuals.

Purpose of the Study:

  • To investigate whether obese women with PE exhibit higher circulating levels of VFM-preferential adipokines compared to obese women without PE.
  • To explore the association between VFM-associated adipokines and PE, independent of BMI.

Main Methods:

  • A case-control study comparing obese (BMI ≥ 30 kg/m²) and normal-weight (BMI 18.4-24.9 kg/m²) women with and without PE.
  • Plasma concentrations of VFM-preferential adipokines (visfatin, resistin), overall adiposity markers (leptin, adiponectin), and inflammatory cytokines were measured.

Main Results:

  • Obese women with PE showed significantly higher levels of VFM-associated adipokines and cytokines (visfatin, resistin, IFN gamma, IL-6, IL-1beta, IL-2) compared to obese controls.
  • Interestingly, both obese and normal-weight PE cases had similar levels of VFM-adipokines and cytokines, suggesting a BMI-independent association.

Conclusions:

  • Preeclampsia is associated with elevated concentrations of VFM-preferential adipokines in both normal-weight and obese women.
  • Visceral fat mass may play a crucial role in the development of PE, potentially offering a more precise risk indicator than BMI alone.
