The shape of the levator hiatus does not affect the link between hiatal area and pelvic organ prolapse (POP). Hiatal ballooning is strongly associated with POP symptoms and signs.
Area of Science:
- Urogynecology
- Pelvic Floor Disorders
- Medical Imaging
Background:
- Levator ani trauma and hiatal overdistension are linked to female pelvic organ prolapse (POP).
- The specific role of levator hiatus shape in POP has not been previously investigated.
Purpose of the Study:
- To examine the association between levator hiatus configuration and POP.
- To determine if hiatal shape influences the relationship between hiatal area and POP.
Main Methods:
- Retrospective analysis of 547 women with pelvic floor dysfunction symptoms.
- Utilized four-dimensional translabial ultrasound (TLUS) and Pelvic Organ Prolapse Quantification (POP-Q).
- Measured hiatal dimensions (anteroposterior diameter, coronal diameter, hiatal area) and hiatal shape (APD/CD ratio) at rest and during Valsalva maneuver.
Main Results:
- 44% of women reported prolapse symptoms; 74% had clinically significant POP.
- Hiatal ballooning was observed in 57% of women and strongly correlated with POP signs and symptoms.
- Hiatal area (HA) at rest and Valsalva was associated with significant POP, but hiatal shape did not alter this association.
Conclusions:
- Levator hiatus shape does not appear to influence the relationship between hiatal area and POP.
- Hiatal ballooning is a significant finding associated with POP.

Stability of Equilibrium Configuration
A stable equilibrium occurs when a system tends to return to its original position when given a small displacement, and the potential energy is at its minimum. An example of a stable equilibrium is when a cantilever beam is fixed at one end and a weight is attached to the other end. If the weight...
