Stepped and collaborative care models (SCM) show high guideline adherence for mild depression treatment selection and initiation. However, adherence and treatment consistency decrease significantly with increasing depression severity, especially for severe cases.
Area of Science:
- Mental Health
- Clinical Psychology
- Healthcare Management
Background:
- Stepped and collaborative care models (SCM) are recommended for optimizing patient allocation in depression treatment.
- These models incorporate low-intensity treatments to improve patient management.
Purpose of the Study:
- To investigate the implementation of guideline-adherent treatments within a SCM for depression in routine care.
- To evaluate treatment selection, initialization, and adherence based on depression severity.
Main Methods:
- Analysis of care provider documentation data from 569 patients treated within an SCM.
- Evaluation of guideline-adherent treatment selection and initialization rates.
- Assessment of accordance between selected and initialized treatments for mild, moderate, and severe depression.
Main Results:
- Guideline adherence for treatment selection and initialization was highest in mild depression (91% and 85%).
- Adherence decreased in moderate (68% selection, 54% initialization) and severe depression (59% selection, 19% initialization).
- Significant discrepancies between selected and initialized treatments were found for moderate and severe depression, but not mild.
Conclusions:
- SCMs show potential for improving mild depression care and guideline adherence in treatment selection.
- Treatment initialization and selection-to-initialization consistency vary significantly with depression severity.
- Inadequate treatment of severe depression may stem more from failed treatment initializations than from poor selection choices.
