Summary
The HeartMate 3 (HM3) pump significantly reduces long-term stroke events in advanced heart failure patients compared to the HeartMate II (HMII). Stroke occurrence, regardless of type or severity, predicts poorer 2-year survival rates.
Area of Science:
- Cardiology
- Neurology
- Biomedical Engineering
Background:
- The MOMENTUM 3 study previously indicated fewer strokes with the HeartMate 3 (HM3) left ventricular assist device (LVAD) compared to the HeartMate II (HMII).
- This analysis provides a detailed examination of stroke events over a 2-year period in patients receiving HM3 or HMII support.
Purpose of the Study:
- To comprehensively analyze the longitudinal occurrence, clinical correlates, patterns, and outcomes of stroke events in patients supported by HM3 versus HMII devices.
- To identify predictors of stroke and assess the impact of stroke on long-term survival.
Main Methods:
- A randomized controlled trial comparing HM3 (centrifugal-flow) and HMII (axial-flow) pumps in advanced heart failure patients.
- Multivariable analyses were used to assess baseline and postimplantation correlates of stroke.
- Longitudinal patterns of stroke (hemorrhagic vs. ischemic, disabling vs. nondisabling) and their association with 2-year outcomes were determined.
Main Results:
- In 361 patients, 65 strokes occurred. Long-term stroke incidence (181-730 days) was 3.3 times lower with HM3 (0.04 events/patient-year) versus HMII (0.13 events/patient-year).
- HM3 pump treatment was the sole independent predictor of reduced stroke events; blood pressure and antithrombotic regimens showed no direct association.
- Stroke events significantly reduced 2-year survival (43-57%) compared to patients without stroke (85%).
Conclusions:
- The HM3 pump demonstrates a significant reduction in stroke rates, particularly in the long term (>6 months), compared to the HMII device.
- Stroke occurrence, irrespective of type or severity, is a significant predictor of poor 2-year clinical outcomes in LVAD patients.

Force and Momentum
24.1K
Force and momentum are intimately related. Force acting over time can change momentum, and Newton's second law of motion can be stated in its most broadly applicable form in terms of momentum. Momentum can be applied to systems where the mass is changing, such as rockets, as well as to systems of constant mass. Also, momentum continues to be a key concept in the study of atomic and subatomic particles in quantum mechanics. One can consider systems with varying mass in some detail; however, the...

Linear Momentum
18.2K
The term momentum is used in various ways in everyday language, most of which are consistent with the precise scientific definition. Generally, momentum implies a tendency to continue on course—to move in the same direction; we tend to speak of sports teams or politicians gaining and maintaining the momentum to win. Momentum is also associated with great mass and speed and is often considered when talking about collisions. For example, when rugby players collide and fall to the...

Angular Momentum
814
Angular momentum characterizes an object's rotational motion and is defined as the moment of its linear momentum about a specified point O. When a particle moves along a curved path in the x-y plane, the scalar formulation calculates the magnitude of its angular momentum, utilizing the moment arm (d), representing the perpendicular distance from point O to the line of action of the linear momentum. Despite being scalar in formulation, angular momentum is inherently a vector quantity. Its...

Long-term Depression
33.3K
Long-term depression, or LTD, is one of the ways by which synaptic plasticity—changes in the strength of chemical synapses—can occur in the brain. LTD is the process of synaptic weakening that occurs over time between pre and postsynaptic neuronal connections. The synaptic weakening of LTD works in opposition to synaptic strengthening by long-term potentiation (LTP) and together are the main mechanisms that underlie learning and memory.

Impulse-Momentum Theorem
19.4K
The total change in the motion of an object is proportional to the total force vector acting on it and the time over which it acts. This product is called impulse, a vector quantity with the same direction as the total force acting on the object.
By writing Newton's second law of motion in terms of the momentum of an object and the external force acting on it, and simultaneously using the definition of the impulse vector, it can be shown that the total impulse on an object is equal to its...

Conservation of Momentum: Introduction

The total momentum of a system consisting of N interacting objects is constant in time or is conserved. A system must meet two requirements for its momentum to be conserved:
