Diffusion tensor imaging (DTI) of normal-appearing white matter in multiple sclerosis (MS) may predict disability. Changes in the corpus callosum (CC) over one year did not predict long-term worsening.

Background:

  • Multiple Sclerosis (MS) involves white matter damage not always visible on conventional MRI.
  • Diffusion Tensor Imaging (DTI) detects subtle microstructural changes in normal-appearing white matter (NAWM).

Purpose of the Study:

  • To evaluate the prognostic value and stability of DTI indices in the NAWM of MS patients.
  • To assess DTI's role in predicting disability progression over four years in relapsing-onset MS.

Main Methods:

  • DTI indices (FA, MD, RD, AD) were measured in the corpus callosum (CC) and internal capsule NAWM of 46 MS patients and 10 controls at baseline and 1 year.
  • Focal lesion volume and atrophy were also assessed.
  • Multivariate logistic regression analyzed associations between DTI, volumetric data, and 4-year disability progression.

Main Results:

  • Baseline DTI metrics differed significantly between MS patients and controls.
  • A trend suggested baseline CC DTI indices might associate with disability progression (p < 0.05).
  • One-year changes in CC DTI indices were significant (p < 0.008) but did not predict 4-year worsening; volumetric data showed no clear association.

Conclusions:

  • Aberrant DTI measures in the CC NAWM may offer insights into 4-year disability progression in relapsing-onset MS.
  • The CC is a potential target for DTI monitoring of MS disease activity.
  • Further research is needed to confirm the prognostic potential of DTI in MS.
