Fibrositis, characterized by joint pain, is uncommon in primary hypothyroidism patients. Thyroid hormone replacement improved symptoms in some, but fibrositis prevalence remained low.

Area of Science:

  • Endocrinology
  • Rheumatology
  • Internal Medicine

Background:

  • Primary hypothyroidism is common and can present with non-specific symptoms.
  • Fibrositis (now often referred to as fibromyalgia) involves widespread musculoskeletal pain and tenderness.
  • The overlap in symptoms suggests a potential link between these conditions.

Purpose of the Study:

  • To determine the prevalence of fibrositis in patients with subclinical or biochemical primary hypothyroidism.
  • To assess the impact of thyroid hormone replacement therapy on fibrositis symptoms and tender points.

Main Methods:

  • A cohort of 100 patients with primary hypothyroidism was evaluated.
  • Patients were assessed for joint/muscle pain and the number of tender points (≥11 required for fibrositis diagnosis).

Related Experiment Videos

  • Symptomatic changes and tender point counts were monitored after thyroid hormone replacement.
  • Main Results:

    • Only 5% of the 100 patients met the diagnostic criteria for fibrositis.
    • Nineteen patients reported pain symptoms, but only 5 had sufficient tender points.
    • Ten of the 19 symptomatic patients improved with thyroid hormone replacement, including 3 with fibrositis.

    Conclusions:

    • Fibrositis is infrequently diagnosed in patients with primary hypothyroidism.
    • While some pain symptoms may improve with thyroid hormone replacement, fibrositis itself is not highly prevalent in this cohort.
    • The study highlights the importance of differentiating fibrositis from general hypothyroidism-related pain.
