Area of Science:

  • Dermatology
  • Pediatrics
  • Pathology

Background:

  • Eosinophilic cellulitis is a rare inflammatory condition.
  • Childhood cases are infrequently reported, predominantly in males.

Observation:

  • A 12-year-old diabetic girl presented with violaceous nodules on her thighs and legs, accompanied by muscular weakness.
  • Lesions were not associated with insulin injection sites.

Findings:

  • Skin biopsy revealed dermal foci of eosinophilic and granular collagen degeneration.
  • A characteristic "flame configuration" of histiocytes and eosinophils was noted.
  • No peripheral blood eosinophilia was detected, differentiating it from other eosinophilic conditions.

Implications:

  • This case expands the known spectrum of eosinophilic cellulitis in children, including a female patient.
  • The "flame configuration" in skin biopsy may be a specific diagnostic indicator.
  • Further research is needed to understand the pathogenesis and specific triggers of this condition in pediatric patients.
