Summary

Primary adenomas of the petrous bone are rare, presenting with subtle inner ear symptoms that mimic other benign conditions. Surgical removal via subtotal petrosectomy is effective for these non-malignant tumors.

Background:

Primary adenomas of the petrous bone are exceedingly rare, with limited literature available on their presentation and management.
Differentiating these tumors from other benign lesions of the temporal bone can be challenging due to their slow progression and subtle symptoms.

Observation:

Three cases of primary petrous bone adenomas are presented, alongside one case of an external ear canal adenoma.
Patients exhibited slowly progressive inner ear symptoms, including hearing loss and vertigo, without other neurological deficits.
Radiographic and clinical findings revealed bone destruction, vascular tumor tissue, and cysts, indicative of a benign but invasive process.

Findings:

Histopathological examination confirmed the benign nature of the adenomas, showing no direct tumor invasion or signs of malignancy.
One case suggested a possible association with von Hippel-Lindau disease, given a strong family history and a contralateral lesion.
Subtotal petrosectomy was identified as a successful surgical approach for complete tumor excision.

Implications:

This study highlights the importance of considering primary petrous bone adenomas in the differential diagnosis of slowly progressive inner ear symptoms.
The findings underscore the benign yet destructive potential of these tumors and the efficacy of surgical management.
Further research into the potential genetic links, such as von Hippel-Lindau disease, may improve understanding and diagnostic approaches.
