Summary

Laparoendoscopic single-port surgery (LESS) for totally extraperitoneal (LESS-TEP) hernioplasty offers improved cosmetic outcomes compared to conventional (CTEP) surgery. While operative time is longer, LESS-TEP is a safe and feasible option for inguinal hernia repair.

Background:

  • Inguinal hernia repair is a common surgical procedure.
  • Conventional totally extraperitoneal (CTEP) hernioplasty is a standard technique.
  • Laparoendoscopic single-port surgery (LESS) for totally extraperitoneal (LESS-TEP) hernioplasty is an emerging approach.

Purpose of the Study:

  • To compare the surgical outcomes of LESS-TEP hernioplasty versus CTEP hernioplasty.
  • To evaluate postoperative pain, cosmesis, complications, and hospitalization.
  • To assess the feasibility and safety of LESS-TEP for inguinal hernia repair.

Main Methods:

  • Retrospective analysis of 81 LESS-TEP and 88 CTEP cases (March 2015 - May 2018).
  • Postoperative outcomes assessed using visual-analog pain scale and cosmesis evaluation.
  • Complications and surgical experience were analyzed.

Main Results:

  • LESS-TEP showed significantly longer operative times (P < .05).
  • Cosmetic outcomes were significantly better in the LESS-TEP group (P < .05).
  • No significant differences in hospitalization, complications, pain, or costs were observed between groups (P > .05).

Conclusions:

  • LESS-TEP is a feasible and safe alternative for inguinal hernia repair.
  • Improved cosmetic results with smaller scars are a key advantage of LESS-TEP.
  • LESS-TEP is not yet a universally necessary alternative to CTEP, especially for less experienced surgeons.
