Background:

Multifocal intraosseous hemangioma is a rare condition affecting multiple skeletal sites.
Accurate diagnosis and treatment selection are challenging in multifocal intraosseous hemangioma.

Observation:

A 49-year-old woman presented with severe lumbar pain.
Imaging revealed hemangioma lesions in the ribs, lumbar vertebrae, ilium, pubis, and femur.
Iliac bone biopsy confirmed the diagnosis of intraosseous hemangioma.

Findings:

Radiotherapy was chosen as the initial treatment for the symptomatic lumbar spine.
The patient experienced no recurrence at the one-year follow-up.

Implications:

Careful selection of biopsy sites is crucial for diagnosing multifocal intraosseous hemangioma.
Radiotherapy is an effective treatment for symptomatic vertebral hemangiomas when neurological deficits are absent.
