1Department of Gynaecology, St Vincent's Hospital, Darlinghurst, New South Wales, Australia.
A genital lump in an 82-year-old woman was diagnosed as chronic lymphocytic leukemia (CLL). This rare presentation highlights the importance of considering CLL in extranodal sites, even with stable disease.
Area of Science:
- Oncology
- Hematology
- Gynecology
Background:
- Chronic lymphocytic leukemia (CLL) is a common B-cell malignancy in adults.
- Extranodal involvement of CLL is typically associated with disease progression.
- Genital involvement in CLL is uncommon, particularly in stable disease.
Observation:
- An 82-year-old woman with a history of stable CLL presented with a labial lump.
- The patient experienced hypogammaglobulinemia and recurrent infections, managed with intravenous immunoglobulin.
- A biopsy of the labial mass confirmed CLL infiltration.
Findings:
- The labial lump represented extranodal involvement of CLL.
- This finding occurred despite the absence of clinical evidence of disease progression.
Implications:
- This case underscores the potential for extranodal CLL presentation in the genital area.
- It emphasizes the need for thorough investigation of new masses in CLL patients, irrespective of overall disease status.
- Further research into the mechanisms and management of extranodal CLL is warranted.

Additional contributing factors include inadequate dental hygiene, mouth breathing, recurring tonsillitis, allergic rhinitis, laryngopharyngeal reflux, and exposure to smoke, chemicals, and other environmental pollutants. Allergic reactions to pollen, mold, and pet dander, chronic cough, excessive voice usage,...
