Abstract

Insights

Avelumab demonstrated durable responses and promising survival in patients with metastatic melanoma. This immunotherapy showed an acceptable safety profile, with manageable side effects in the JAVELIN Solid Tumor trial.

Area of Science:

  • Oncology
  • Immunotherapy
  • Clinical Trials

Background:

  • Phase 1b data from the JAVELIN Solid Tumor trial (NCT01772004) are presented.
  • Focus on patients with unresectable stage IIIC or IV melanoma that progressed after at least one line of therapy for metastatic disease.

Purpose of the Study:

  • To evaluate the efficacy and safety of avelumab, a human anti-PD-L1 antibody, in patients with previously treated metastatic melanoma.
  • Assess objective response rate (ORR), progression-free survival (PFS), overall survival (OS), and safety profile.

Main Methods:

  • 51 patients received avelumab (10 mg/kg).
  • Data collected included ORR, PFS, OS, and safety assessments.
  • Follow-up was conducted for a median of 24.2 months.

Main Results:

  • Confirmed ORR was 21.6% (95% CI, 11.3-35.3), with a complete response rate of 7.8%.
  • Median PFS was 3.1 months (95% CI, 1.4-6.3) and median OS was 17.2 months (95% CI, 6.6-not estimable).
  • Meaningful activity observed in subgroups: non-ocular melanoma (ORR 31.4%), PD-L1-positive tumors (ORR 42.1%), and post-ipilimumab therapy (ORR 30.8%).
  • 76.5% experienced treatment-related adverse events (TRAEs), most commonly infusion-related reactions and fatigue. Grade 3 TRAEs occurred in 7.8%, and immune-related TRAEs in 9.8% (all grade 1/2). No grade 4 TRAEs or treatment-related deaths.

Conclusions:

  • Avelumab demonstrates durable responses and promising survival outcomes in previously treated metastatic melanoma patients.
  • The safety profile of avelumab is acceptable, with a low incidence of severe adverse events.
  • Avelumab represents a viable treatment option for this patient population.

Phase Diagrams02:39

Phase Diagrams

A phase diagram combines plots of pressure versus temperature for the liquid-gas, solid-liquid, and solid-gas phase-transition equilibria of a substance. These diagrams indicate the physical states that exist under specific conditions of pressure and temperature and also provide the pressure dependence of the phase-transition temperatures (melting points, sublimation points, boiling points). Regions or areas labeled solid, liquid, and gas represent single phases, while lines or curves represent...

Metallic Solids02:37

Metallic Solids

Metallic solids such as crystals of copper, aluminum, and iron are formed by metal atoms. The structure of metallic crystals is often described as a uniform distribution of atomic nuclei within a “sea” of delocalized electrons. The atoms within such a metallic solid are held together by a unique force known as metallic bonding that gives rise to many useful and varied bulk properties.
All metallic solids exhibit high thermal and electrical conductivity, metallic luster, and malleability....

Structures of Solids02:22

Structures of Solids

Solids in which the atoms, ions, or molecules are arranged in a definite repeating pattern are known as crystalline solids. Metals and ionic compounds typically form ordered, crystalline solids. A crystalline solid has a precise melting temperature because each atom or molecule of the same type is held in place with the same forces or energy. Amorphous solids or non-crystalline solids (or, sometimes, glasses) which lack an ordered internal structure and are randomly arranged. Substances that...

Phase Transitions02:31

Phase Transitions

Whether solid, liquid, or gas, a substance's state depends on the order and arrangement of its particles (atoms, molecules, or ions). Particles in the solid pack closely together, generally in a pattern. The particles vibrate about their fixed positions but do not move or squeeze past their neighbors. In liquids, although the particles are closely spaced, they are randomly arranged. The position of the particles are not fixed—that is, they are free to move past their neighbors to...

Clinical Trials: Overview

Clinical development focuses on how the drug will interact with the human body and encompasses four key phases of clinical trials, each serving a specific purpose in assessing the safety and effectiveness of new drugs. These phases overlap and build upon one another. Phase I involves a small group of healthy volunteers (typically 20-80 individuals) or, in cases where significant toxicity is expected, patients with the targeted disease, such as cancer or AIDS. The volunteers are tested for...

Trial and Error and Algorithm01:12

Trial and Error and Algorithm

A problem-solving strategy is a plan of action used to find a solution. Different strategies have distinct action plans. Trial and error involves trying different solutions until one works. For instance, to fix a broken printer, you might check ink levels, ensure the paper tray isn't jammed, and verify the printer's connection to your laptop. This method can be time-consuming but is commonly used. Thomas Edison, for example, used trial and error to find a suitable filament for the light...
