Background:

Madelung disease (MD), also known as benign symmetric lipomatosis, is a rare disorder associated with chronic alcohol abuse.
MD is often under-recognized and under-reported due to physician unawareness.

Observation:

A 45-year-old male with a ten-year history of heavy alcohol consumption (over 120g/day) presented with fatigue, abdominal distension, and progressive bilateral breast enlargement.
Clinical presentation, laboratory results, and radiographic findings led to the diagnosis of MD.

Findings:

The patient was treated with alcohol abstinence and supportive therapy.
Following treatment, the patient's condition stabilized with symptom improvement.

Implications:

Physicians should consider MD in patients with bilateral breast enlargement and a history of alcoholic liver disease.
Prompt diagnosis and emphasizing alcohol abstinence are key to managing Madelung disease.
