His bundle pacing (HBP) with cardiac resynchronization therapy (CRT) significantly improved heart failure symptoms and ejection fraction in patients with permanent atrial fibrillation and bundle branch block. This novel approach offers a promising alternative when other treatments fail.

Background:

  • Permanent atrial fibrillation (AF) with heart failure (HF) and bundle branch block (BBB) poses treatment challenges.
  • Conventional therapies may be insufficient for patients with significant QRS widening and impaired left ventricular ejection fraction (LVEF).
  • His bundle pacing (HBP) combined with cardiac resynchronization therapy (CRT) is an emerging technique for select patient populations.

Purpose of the Study:

  • To evaluate the efficacy of implantable cardioverter defibrillator (ICD)/CRT systems utilizing His bundle pacing (HBP) in patients with permanent AF, HF, and BBB.
  • To assess the impact of HBP on hemodynamic parameters, clinical status, and left ventricular function.

Main Methods:

  • Fourteen consecutive patients with permanent AF, HF, BBB (QRS > 130 ms), and low LVEF underwent ICD/CRT system implantation with HBP.
  • Patients were followed to assess the effectiveness of HBP in managing HF symptoms and improving cardiac function.
  • Key parameters monitored included LVEF, QRS duration, ventricular dimensions, and New York Heart Association (NYHA) functional class.

Main Results:

  • The mean LVEF significantly improved from 24% to 38% (P=0.0015) over a 14.4-month follow-up.
  • Mean QRS duration shortened from 159 ms to 128 ms (P=0.016), and ventricular dimensions decreased significantly.
  • 92.3% of patients showed significant improvement in NYHA functional class, indicating substantial clinical benefit.

Conclusions:

  • His bundle pacing integrated with ICD/CRT systems effectively narrows QRS duration and improves left ventricular mechanical function.
  • This approach leads to significant clinical improvement in patients with permanent AF, HF, and BBB refractory to other treatments.
  • Optimization of pacing configuration and pharmacological therapy is crucial for maximizing the benefits of HBP in CRT.
