Patients with inflammatory bowel disease (IBD) have a higher likelihood of developing bipolar disorder (BD). This study investigated the association between IBD and BD, finding a significant link in Taiwanese adults.
Area of Science:
- Gastroenterology and Psychiatry
- Public Health Research
- Epidemiology
Background:
- Emerging evidence suggests shared biological pathways between inflammatory bowel disease (IBD) and bipolar disorder (BD).
- Existing research on the IBD-BD association is limited and presents conflicting findings, particularly in non-Western populations.
- This study addresses the need for further investigation into the potential link between IBD and BD.
Purpose of the Study:
- To investigate the association between inflammatory bowel disease (IBD) and bipolar disorder (BD).
- To evaluate the prevalence of BD in patients diagnosed with IBD using a nationwide database.
- To provide epidemiological data on the comorbidity of IBD and BD in Taiwan.
Main Methods:
- A cross-sectional study utilizing the National Health Insurance Research Database in Taiwan.
- Inclusion of 3590 patients with IBD and 14,360 propensity score-matched controls without IBD.
- Conditional logistic regression analysis to assess the association between BD and IBD.
Main Results:
- Bipolar disorder (BD) was diagnosed in 0.72% of IBD patients versus 0.34% of controls.
- Patients with IBD had a significantly higher adjusted odds ratio (OR) of 2.10 (95% CI: 1.30–3.38) for BD.
- Ulcerative colitis patients showed a similar increased adjusted OR of 2.23 (95% CI: 1.31–3.82) for BD.
Conclusions:
- Patients diagnosed with inflammatory bowel disease (IBD) exhibit a greater likelihood of having bipolar disorder (BD).
- The findings suggest a significant association between IBD and an increased risk of BD.
- Further research is warranted to elucidate the causal relationship and underlying mechanisms.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...

A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...
