Summary

Type 2 idiopathic macular telangiectasia (MacTel) shows prolonged retinal fluorescence lifetimes, particularly temporal to the fovea. This finding may help assess disease progression and photoreceptor loss in MacTel patients.

Background:

  • Type 2 idiopathic macular telangiectasia (MacTel) is a rare, bilateral neurodegenerative disease.
  • MacTel causes central vision loss due to alterations in the macular capillary network.

Purpose of the Study:

  • To quantify disease-specific retinal fluorescence lifetime patterns in MacTel patients.
  • To utilize fluorescence lifetime imaging ophthalmoscopy (FLIO) for MacTel assessment.

Main Methods:

  • 14 MacTel patients and 14 age-matched controls underwent FLIO.
  • Retinal fluorescence lifetimes (Tm) were measured in short and long spectral channels.
  • Clinical data, fundus imaging, OCT, and macular pigment optical density (mpOD) were analyzed.

Main Results:

  • Significantly prolonged Tm temporal to the fovea in MacTel patients compared to controls.
  • Prolonged lifetimes showed a crescent/ring pattern and correlated with decreased mpOD.
  • Follow-up revealed a 22% increase in short-channel Tm over 2.1 years in temporal MacTel.

Conclusions:

  • Retinal fluorescence lifetimes exhibit characteristic patterns in MacTel Type 2.
  • FLIO provides insights into macular pigment and photoreceptor loss.
  • Lifetime prolongation correlates with disease severity, aiding MacTel progression assessment.
