Maximal exercise diminishes coagulation capacity in severe hemophilia A (HA) patients, unlike healthy individuals. Exercise-induced von Willebrand factor (VWF) increases do not prolong factor concentrate effects in HA patients.
Area of Science:
- Hematology
- Exercise Physiology
- Coagulation Disorders
Background:
- Previous research indicated reduced global coagulation after maximal exercise in severe hemophilia A (HA).
- This study aimed to validate findings in a larger cohort and explore exercise's impact on factor concentrate efficacy.
Purpose of the Study:
- To validate diminished coagulation capacity post-exercise in severe HA.
- To investigate if exercise-induced von Willebrand factor (VWF) levels prolong factor concentrate effects.
Main Methods:
- Maximal physical exercise was performed by 10 severe HA patients and 10 healthy controls.
- Coagulation parameters, including VWF and FVIII:C, were measured pre- and post-exercise at multiple time points.
Main Results:
- Healthy controls showed increased coagulation capacity post-exercise; severe HA patients did not.
- VWF levels rose significantly in both groups, but FVIII:C only increased in controls.
- No significant difference in FVIII pharmacokinetics was observed with or without exercise.
Conclusions:
- Findings emphasize Factor VIII's role in exercise-induced coagulation enhancement.
- Exercise-induced VWF elevation does not appear to prolong factor concentrate efficacy in severe HA patients.
