[Effect of peripheral vasodilators on the local contractile function of the myocardium in patients with
Insights
Echocardiography revealed regional myocardial contractile dysfunctions in patients with postinfarction cardiosclerosis and heart failure. Peripheral vasodilators improved contractility in hypokinetic and intact myocardial areas.
Area of Science:
- Cardiology
- Medical Imaging
- Physiology
Context:
- Investigating postinfarction cardiosclerosis with late-stage heart failure.
- Assessing regional myocardial contractility using echocardiography.
- Analyzing 92 patients with established heart conditions.
Purpose:
- To explore regional myocardial contractility in patients with postinfarction cardiosclerosis and heart failure.
- To identify patterns of contractile dysfunction and compensatory hyperfunction.
- To evaluate the effect of peripheral vasodilators on myocardial contractility.
Summary:
- Echocardiography demonstrated regional myocardial contractile dysfunctions (hypo-, a-, dyskinesia) in patients with postinfarction cardiosclerosis and heart failure.
- The most significant asynergy was observed in anterior postinfarction scar regions.
- Intact myocardial segments exhibited compensatory hyperfunction.
Impact:
- Peripheral vasodilators showed a positive effect, improving contractility in hypokinetic and intact myocardial areas.
- Findings contribute to understanding myocardial mechanics in heart failure post-myocardial infarction.
- Highlights potential therapeutic targets for improving cardiac function in this patient population.
Abstract:
Echocardiography was used to explore regional myocardial contractility in 92 patients with postinfarction cardiosclerosis complicated by heart failure developed in the late postinfarction period. All the patients were shown to display regional myocardial contractile dysfunctions as hypo-, a-, and dyskinesia accompanied with hyperfunction of intact myocardial segments. The most severe asynergy was present in anterior postinfarction scars. Peripheral vasodilators were found to improve contractility in the hypokinetic and intact areas of the myocardium.
