Background:

  • Interstitial lung abnormalities (ILA) are radiologic findings on CT scans, often indicating early pulmonary fibrosis.
  • While ILA progression is known, the association between specific imaging patterns and mortality risk remains unclear.

Purpose of the Study:

  • To investigate the relationship between ILA imaging patterns and the risk of mortality.
  • To determine if specific imaging patterns predict ILA progression.

Main Methods:

  • Chest CT scans from 5,320 participants in the AGES-Reykjavik Study were analyzed for ILA and imaging patterns.
  • Progression was assessed in 3,167 participants; logistic regression and Cox proportional hazards models were used for analysis.

Main Results:

  • Over 5 years, 10% of participants had ILA, with 73% showing imaging progression.
  • The definite fibrosis pattern showed the highest risk of progression (OR 8.4).
  • Probable usual interstitial pneumonia (HR 1.7) and usual interstitial pneumonia (HR 3.9) patterns were linked to increased mortality risk.

Conclusions:

  • Imaging patterns in ILA can predict progression.
  • Specific radiologic patterns in ILA are associated with an elevated risk of mortality.
  • ILA imaging patterns aid in identifying individuals at higher risk for progression and early death.
