A case report of primary osteosarcoma originating from kidney

Primary renal osteosarcoma is rare and aggressive. This case shows long-term survival in an advanced-stage patient treated with surgery, chemotherapy, and targeted therapy, possibly due to specific gene mutations.

Background:
- Primary osteosarcoma of the kidney is an exceptionally rare renal neoplasm, with fewer than 30 cases documented since 1936.
- This tumor type is associated with a high propensity for metastasis and a poor prognosis.
Observation:
- A 48-year-old female presented with a massive (21cm x 18cm x 11cm) left kidney osteosarcoma with stage IV lung metastasis.
- Imaging revealed a large, partly calcified retroperitoneal mass with pulmonary metastases.
Findings:
- The patient underwent radical nephrectomy followed by chemotherapy (ifosfamide, cisplatinum, pirarubicin) and anlotinib targeted therapy.
- Postoperative follow-up of 26 months showed no recurrence, complications, or progression of pulmonary metastasis.
Implications:
- This case demonstrates a unique instance of long-term disease control in advanced-stage primary renal osteosarcoma.
- Genetic analysis revealed three mutated genes, potentially explaining the patient's favorable response to treatment and extended survival.
