Gene Conversion02:08

Gene Conversion

10.6K
Other than maintaining genome stability via DNA repair, homologous recombination plays an important role in diversifying the genome. In fact, the recombination of sequences forms the molecular basis of genomic evolution. Random and non-random permutations of genomic sequences create a library of new amalgamated sequences. These newly formed genomes can determine the fitness and survival of cells. In bacteria, homologous and non-homologous types of recombination lead to the evolution of new...

Conversion of Units01:36

Conversion of Units

36.1K
Sometimes, there is a need to convert from one unit to another one. For instance, reading a cookbook in which quantities are expressed in units of liters or ounces may require conversion of quantities to cups. Or, when looking up directions on how to get to a location, we may be interested to know how many miles we are going to walk. In this case, we would have to convert units of feet or meters to miles.
The first step in the unit conversion is to list the given units and the units required...

Conversion of Alcohols to Alkyl Halides02:48

Conversion of Alcohols to Alkyl Halides

8.4K
This lesson delves into the conversion of alcohols to corresponding alkyl halides and the mechanism of action for different reagents. Typically, the hydroxyl group is first protonated to convert it to a stable leaving group. Consequently, based on the starting alcohol, the mechanism undergoes either of the nucleophilic substitution routes, SN1 or SN2. Tertiary alkyl halides are made using the two-step SN1 mechanism that occurs via a carbocation intermediate, which is stabilized by...

Drug Dosing: Infants and Children

292
Pediatric patient dosages diverge from adults due to disparities in body surface area, total body water, and extracellular fluid per kilogram of body weight. The dosing regimen considers the variations in pharmacokinetics and pharmacology across distinct age groups, encompassing preterm newborns, infants, young children, older children, and adolescents. Calculation of pediatric patient doses is predicated on determining body surface area, which exhibits a superior correlation with the child's...

Load along a Single Axis01:29

Load along a Single Axis

645
In structural engineering, the analysis of beams subjected to varying loads is a critical aspect of understanding the behavior and performance of these structural elements. A common scenario involves a beam subjected to a combination of different load distributions.
Consider a beam of length L subjected to a varying load, which is a combination of parabolic and trapezoidal load distribution along the x-axis. In this case, it is essential to determine the resultant loads, their locations, and...

Summary

Single-incision laparoscopic appendectomy (SILA) is safe and efficient for pediatric appendicitis, with faster operative times. Obese or male patients, or those with perforation, may require conversion to multiport appendectomy.

Area of Science:

  • Pediatric Surgery
  • Minimally Invasive Surgery

Background:

  • Single-incision laparoscopic appendectomy (SILA) is widely adopted for pediatric appendectomies.
  • Factors influencing conversion from SILA to multiport appendectomy require identification.

Purpose of the Study:

  • To identify predictors of conversion from SILA to multiport appendectomy.
  • To compare outcomes between SILA and conventional multiport appendectomy (CLA) in children.

Main Methods:

  • Retrospective review of 1001 pediatric appendectomies (2012-2017).
  • Analysis of operative time, length of stay (LOS), complications, and readmission rates.
  • Inclusion of demographic data (age, sex, BMI) and perforation status.

Main Results:

  • 95.9% of appendectomies were initiated as SILA, with 92.2% completed successfully.
  • Conversion to multiport appendectomy was associated with male sex, higher BMI, and perforated appendicitis.
  • SILA demonstrated significantly faster operative times compared to CLA, with similar LOS and readmission rates.

Conclusions:

  • SILA is a safe and efficient method for pediatric appendicitis treatment, without increased morbidity.
  • Counseling for potential conversion is advised for obese children, males, or those with perforation.
  • Consideration for CLA may be appropriate for specific high-risk pediatric appendicitis cases.
