Summary

This study demonstrates that long-term high-density electroencephalography (EEG) is a well-tolerated method for capturing epileptic seizures. While it shows promise, it did not significantly improve sublobar localization compared to standard EEG in this small trial.

Background:

  • High-density EEG (hd-EEG) is clinically restricted due to placement challenges and technical demands.

Purpose of the Study:

  • To evaluate the feasibility and tolerability of long-term hd-EEG using a 128-channel cap.
  • To assess the efficacy of hd-EEG in capturing seizures and improving source localization in refractory epilepsy.
  • To compare hd-EEG localization with standard 10-20 EEG placement.

Main Methods:

  • A 128-channel neoprene cap with dielectric paste was used for long-term EEG recording in 7 patients.
  • Daily impedance checks and lead reapplication ensured recording quality (impedance < 10 kΩ).
  • Electrode application took ~1 hour, with ~30 minutes daily for maintenance.

Main Results:

  • Successful long-term hd-EEG recordings captured seizures in 6 out of 7 patients.
  • EEG source localization was achieved in 6 patients, consistent with standard EEG within the sublobar region.
  • Mild headaches were reported by 3 patients but did not necessitate discontinuation.

Conclusions:

  • Long-term hd-EEG with dielectric paste is well-tolerated and effective for capturing interictal and ictal data.
  • In this cohort, hd-EEG did not offer a significant advantage in sublobar localization over standard EEG.
  • Further research is needed to fully elucidate the benefits of hd-EEG for precise epilepsy localization.
