Summary

Atrial fibrillation (AF) is common in acute coronary syndrome (ACS) patients. Most eligible AF patients received oral anticoagulants, with novel oral anticoagulants (NOACs) preferred over vitamin K antagonists.

Background:

  • Acute coronary syndrome (ACS) is a critical condition requiring comprehensive management.
  • Atrial fibrillation (AF) is a common comorbidity in ACS patients, increasing the risk of thromboembolic events.

Purpose of the Study:

  • To determine the prevalence of atrial fibrillation (AF) in adult patients hospitalized with acute coronary syndrome (ACS).
  • To evaluate the utilization patterns of antithrombotic agents, including oral anticoagulants and antiplatelet therapies, in ACS patients with AF.

Main Methods:

  • A consecutive cohort of 1155 ACS patients was enrolled from two Moscow-based percutaneous coronary intervention centers.
  • AF was diagnosed in 204 patients (17.7%), with risk stratification using CHA2DS2-VASc and HAS-BLED scores.
  • Data analysis was performed using StatSoft Statistica 10.0 and IBM SPSS Statistics v.23.

Main Results:

  • The overall prevalence of AF was 17.7% (13.6% diagnosed, 4.1% undiagnosed).
  • Of 179 discharged AF patients, 83.8% eligible for oral anticoagulant therapy received it, with novel oral anticoagulants (NOACs) predominantly used (80.0%).
  • Triple therapy (oral anticoagulant plus two antiplatelet agents) was common in patients undergoing percutaneous coronary intervention (PCI).

Conclusions:

  • Atrial fibrillation is highly prevalent in patients with acute coronary syndrome.
  • The observed preference for NOACs indicates physician awareness of their role in preventing thromboembolic complications in AF patients.
