Background:

  • Postural control (PC) deteriorates with neuromuscular fatigue, altitude, and sleep deprivation, common in mountain ultra-marathons (MUMs).
  • Regulatory mechanisms during MUMs are known, but the time course of PC changes remains unclear.

Purpose of the Study:

  • To investigate the time course of postural control (PC) during an extreme mountain ultra-marathon (MUM).
  • To test the hypothesis that PC alteration does not increase linearly during a MUM.

Main Methods:

  • 16 participants (age 45.1 ± 9.6 years) underwent bipedal posturography every ~50 km during a MUM.
  • Analyses included traditional posturography and stabilogram diffusion analysis (SDA).
  • Subjective fatigue, sleep feeling, and pain were assessed using a visual analog scale (VAS).

Main Results:

  • Postural control parameters significantly increased until 100 km, confirmed by SDA.
  • The short-term effective diffusion coefficient and critical point also significantly increased up to 100 km.
  • From 100 km to 200 km, most PC parameters decreased, with significant increases in the short-term effective diffusion coefficient and critical point, indicating a bi-phasic response.

Conclusions:

  • Postural control degradation progresses until 100 km in extreme MUMs.
  • Compensatory mechanisms appear to mitigate further posture degradation after 100 km.
  • This bi-phasic response offers insights into coping with extreme fatigue during endurance events.
