Isolated acromioclavicular joint tuberculosis is a rare condition that can be easily misdiagnosed. Early recognition and treatment are crucial for preventing long-term complications.

- Imaging showed acromioclavicular joint destruction; histopathology confirmed tuberculosis.
Findings:
- Surgical debridement, synovectomy, and abscess drainage were performed.
- The patient recovered well with postoperative antitubercular therapy.
Implications:
- Increased awareness of this rare presentation aids early diagnosis.
- Consider osteoarticular tuberculosis in atypical shoulder presentations, especially in endemic regions.
