Summary

Area of Science:

  • Uro-oncology
  • Nephrology
  • Pathology

Background:

  • Transitional cell carcinoma (TCC) of the renal pelvis is a rare malignancy.
  • Bilateral renal pelvic tumors are even rarer, posing unique management challenges.

Observation:

  • A 42-year-old male with a prior left nephroureterectomy for high-grade TCC presented with asymptomatic hematuria.
  • Diagnostic evaluation revealed a synchronous TCC in the right renal pelvis.

Findings:

  • Histological examination confirmed Grade 2 TCC in the right renal pelvis, necessitating a right nephroureterectomy.
  • Postoperatively, the patient initiated intermittent maintenance hemodialysis due to impaired renal function.
  • At 10 months follow-up, the patient remains disease-free with no recurrence or metastasis.

Implications:

  • This case highlights the potential for bilateral TCC of the renal pelvis and the importance of vigilant surveillance.
  • Management requires a multidisciplinary approach, considering surgical options and renal replacement therapy.
  • Reviewing 28 cases of bilateral renal pelvic tumors provides insights into incidence, prognosis, and optimal management strategies, including postoperative hemodialysis.
