    Background:

    • The Model for Understanding Success in Quality (MUSIQ) is a framework identifying contextual factors influencing quality improvement (QI) initiatives.
    • Previous research suggests subjective outcome measures in QI are associated with specific MUSIQ factors.
    • Understanding the relationship between contextual factors and objective QI success is crucial for healthcare delivery.

    Purpose of the Study:

    • To investigate the association between MUSIQ contextual factors and objective measures of QI success in an ambulatory, urban primary care network.
    • To compare objective QI outcomes with subjective, self-reported outcomes in relation to MUSIQ factors.

    Main Methods:

    • A cross-sectional survey study was conducted across 21 primary care sites.
    • A modified MUSIQ survey tool was administered to QI team members.
    • Objective QI success was measured by goal achievement for adult diabetes care (HbA1c < 8%) and pediatric vaccination rates.

    Main Results:

    • No MUSIQ contextual factors were significantly associated with either the adult or pediatric objective measures of QI project success.
    • A post hoc analysis revealed that objective and subjective measures of success were often not associated or negatively correlated.

    • Contextual factors, as measured by the MUSIQ framework, were not associated with objective measures of QI outcomes in this primary care setting.
    • The findings challenge previous research that found associations between MUSIQ factors and subjective QI success.
    • Further research may be needed to explore other potential drivers of objective QI success in primary care.
