  • Post-shunt, she developed progressive neurological deficits including extremity numbness, gait disturbance, and cognitive decline.
  • The endodermal cyst, initially unrecognized, became evident at the craniocervical junction with multiple pontomedullary cistern lesions.

Findings:

  • Subtotal resection revealed multiple cysts with milky fluid and thickened arachnoid membranes.
  • Post-surgery, neurological symptoms partially improved, but cognitive decline persisted.
  • Recurrence occurred two months post-operation, with no further surgical intervention due to the patient's poor condition.

Implications:

  • Endodermal cysts can rarely cause nonobstructive hydrocephalus, potentially mimicking recurrent meningitis.
  • These lesions are often occult on initial imaging and difficult to resect completely due to adhesions.
  • This case underscores the diagnostic and therapeutic challenges posed by intracranial endodermal cysts.
