Low fractional flow reserve (FFR) after stenting often lacks clear causes. Intravascular ultrasound in the FFR SEARCH registry revealed significant luminal narrowing in many cases, particularly smaller minimal stent areas, explaining low post-PCI FFR.

- Intravascular ultrasound identified focal luminal narrowing as a significant factor in cases with low post-PCI FFR.
- Smaller minimal stent area is associated with reduced post-PCI FFR.
