Summary

A new 5-level version of the EQ-5D-Y (EuroQol 5-Dimensions Youth) was developed and preferred by participants. This enhanced health status measure requires further validation for children and adolescents.

Background:

  • The EQ-5D-Y (EuroQol 5-Dimensions Youth) is a standard tool for measuring child and adolescent health status.
  • The original EQ-5D-Y uses three severity levels (3L) per dimension.
  • There is a need for more granular health status measures in pediatric populations.

Purpose of the Study:

  • To develop a descriptive system for the EQ-5D-Y with an increased number of severity levels (4L and 5L).
  • To assess the comprehensibility and feasibility of the modified EQ-5D-Y versions.
  • To identify participant preference between 4-level and 5-level versions.

Main Methods:

  • A multinational study across Germany, Spain, Sweden, and the UK.
  • Phase 1 involved literature review and focus groups to generate labels, followed by sorting and response scaling interviews with 255 participants (aged 8-15).
  • Phase 2 involved cognitive interviews to test preliminary 4L and 5L versions with healthy children and those with health conditions.

Main Results:

  • A total of 233 labels were generated in Phase 1.
  • Labels effectively covered a range of health severities across all participating countries.
  • The 5-level version (5L) was preferred over the 4-level version (4L) by 68-88% of participants in Phase 2.

Conclusions:

  • A multinational study successfully developed an extended EQ-5D-Y version with 5 severity levels (EQ-5D-Y-5L).
  • This new EQ-5D-Y-5L version requires further evaluation of its psychometric properties.
  • Future research should compare the performance of the EQ-5D-Y-5L against the original EQ-5D-Y-3L.
