Summary
Pigmented villonodular synovitis (PVNS) is a rare joint condition. This case report highlights a rare subtalar joint PVNS, successfully treated with surgical excision, with no recurrence after three years.

Background:
- Pigmented villonodular synovitis (PVNS) is a rare, benign neoplastic proliferation of the synovial joint.
- While commonly affecting large joints like the knee, PVNS is exceptionally rare in the subtalar joint.
Observation:
- A 60-year-old patient presented with a two-year history of swelling on the outer aspect of the right foot.
- Standard radiographs and biological tests showed no abnormalities.
- Magnetic resonance imaging (MRI) suggested PVNS, which was confirmed by surgical biopsy and excision.
Findings:
- The patient underwent surgical excision of the subtalar PVNS.
- Post-operative follow-up was uncomplicated.
- No recurrence of the subtalar PVNS was observed after three years.
Implications:
- Subtalar PVNS is an extremely rare presentation of this condition.
- Surgical synovectomy is the primary treatment for PVNS.
- Prognosis is linked to the extent of osteocartilaginous damage and the completeness of surgical excision.
