Summary

  • Alternative options include hemispherical threaded cups designed for enhanced primary stability.

Purpose of the Study:

  • To compare the clinical and radiographic outcomes of a 3rd-generation threaded hip cup against a press-fit cup with a well-established track record.
  • To evaluate the efficacy and safety of different uncemented acetabular component fixation methods in THA.

Main Methods:

  • Retrospective review of 300 consecutive total hip arthroplasties (150 threaded cups, 150 press-fit cups) performed by two surgeons.
  • Minimum follow-up of 27 months, maximum 78 months (mean 52.5 months).
  • Primary endpoint: stable and painless THA. Secondary endpoints: Hip disability and Osteoarthritis Outcome Score (HOOS), and radiographic evaluation.

Main Results:

Conclusions:

  • Both 3rd-generation threaded cups and press-fit cups provide comparable clinical and radiographic results in total hip arthroplasty at mid-term follow-up.
  • While press-fit cups showed a non-significant trend towards earlier loosening, concerns regarding bone loss and revision difficulty with screwed cups persist.
  • Further long-term studies are warranted to fully elucidate the comparative advantages and disadvantages of these acetabular component fixation strategies.
