Anemia in severe aortic stenosis (AS) patients significantly increases risks for death, heart failure, and bleeding. Moderate/severe anemia poses the highest risk, even after aortic valve replacement, suggesting anemia management may improve outcomes.
Area of Science:
- Cardiology
- Hematology
Background:
- Anemia is a common comorbidity in patients with severe aortic stenosis (AS).
- The prognostic impact of anemia in severe AS is not well-defined.
Purpose of the Study:
- To assess the association between anemia and cardiovascular and bleeding outcomes in severe AS patients.
Main Methods:
- Analysis of 3403 patients from the CURRENT AS registry.
- Categorization of anemia into mild (Hb 11.0-12.9 g/dL men/11.0-11.9 g/dL women) and moderate/severe (Hb ≤ 10.9 g/dL).
- Comparison of outcomes between anemic and non-anemic patients, stratified by management (conservative vs. aortic valve replacement).
Main Results:
- Mild and moderate/severe anemia were linked to increased risk of the primary outcome (AS-related death or heart failure hospitalization).
- Moderate/severe anemia showed a significantly increased risk for the primary outcome even in the aortic valve replacement group.
- Moderate/severe anemia was associated with a higher risk of major bleeding in patients under conservative management.
Conclusions:
- Anemia is a significant negative prognostic factor in severe AS.
- The presence and severity of anemia correlate with worse cardiovascular and bleeding outcomes.
- Further research into anemia management strategies is warranted to improve clinical outcomes in severe AS.
