Vagus nerve stimulation (VNS) shows unclear efficacy for treating depression, with no significant differences found in most depression rating scales. Further research is needed to confirm its effectiveness and safety in patients with treatment-resistant depression.

Background:

  • Depression is a widespread mental health condition with limited treatment response in many patients.
  • Vagus nerve stimulation (VNS) is an approved adjunctive therapy for treatment-resistant depression.
  • The clinical effectiveness of VNS for depression remains uncertain.

Purpose of the Study:

  • To systematically review the efficacy and safety of Vagus nerve stimulation (VNS) for depression.
  • To evaluate VNS as an adjunctive treatment for patients with major depressive episodes unresponsive to standard therapies.

Main Methods:

  • A systematic review was conducted following Cochrane Handbook guidelines.
  • Searches were performed in PubMed, Embase, CENTRAL, and Web of Science.
  • Two randomized sham-controlled add-on studies with 255 participants were included; suicidal rate was the primary outcome.

Main Results:

  • No studies reported suicidal rates.
  • Qualitative analysis indicated no significant difference between VNS and sham VNS on HAMD24, HAMD24 improvement, MADRS, or IDS-SR30 scores.
  • A marginal difference was observed in the Beck Depression Inventory score, with a minor effect noted for IDS-SR30 in the VNS group.

Conclusions:

  • The efficacy and safety of Vagus nerve stimulation for depression require further investigation.
  • Current evidence is insufficient to definitively establish VNS as an effective treatment for depression.
  • Additional randomized controlled trials are necessary to confirm the therapeutic benefits of VNS.
