Background:

Broad ligament hernia is a rare cause of internal hernia.
Small bowel obstruction necessitates prompt diagnosis and treatment.

Observation:

A 41-year-old woman presented with nausea and vomiting.
CT scan revealed diffuse small bowel dilatation and a caliber change near the uterus, indicative of incarcerated right broad ligament hernia.

Findings:

Needlescopic surgery using 2- and 3-mm instruments was successfully performed.
The broad ligament defect was repaired with sutures, leading to a 2-day post-operative hospital stay.

Implications:

Needlescopic surgery is a promising minimally invasive approach for broad ligament hernias without bowel ischemia.
This technique avoids the need for abdominal incisions and energy devices, potentially reducing patient recovery time.
